I am Jacqueline Medendorp, psychologist, acting under the company name ‘Psychologische Hulp bij Levensproblemen’, Chamber of Commerce number 54391547. I greatly value the protection of your personal data and I do everything to guarantee your privacy.

In this Privacy Statement, I want to provide clear and transparent information about how I process personal data. In all cases I comply with all applicable laws and regulations, including the General Data Protection Regulation. At the least this means that I:

  • Process your personal data in accordance with the purpose for which it was provided. These goals and types of personal data are described in this Privacy Statement;
  • Limit the processing of your personal data to only those data that are minimally required for the purposes for which they are processed;
  • Ask for your express permission if I need it for the processing of your personal data;
  • Have taken appropriate technical and organizational measures to ensure the security of your personal data;
  • Do not transmit personal data to other parties, unless this is necessary for the purposes for which they were provided;
  • Am aware of your rights regarding your personal data, want to point this out and respect them.

Applicability

This Privacy Statement applies to the processing of personal data of:

1. Personal data of visitors of the website medendorp-psychologen.nl

Data being processed

Some data resulting from one or more visits of the website medendorp-psychologen.nl are permanently saved, but anonymously. The data will therefore never be traceable to a person or organization.

Cookies

The website medendorp-psychologen.nl makes use of technical, functional and analytical cookies. Cookies are small text files that are placed by a website page on the computer, tablet or smartphone of you as a visitor. These cookies are necessary for proper technical operation of the website, for your ease of use and for improving and optimizing the website. As a visitor, you can set your browser so that cookies are not placed or are partially allowed. In the latter case, it is possible to select which websites may place functional cookies. It will then be prohibited for all other websites. This option is provided by the most common modern browsers. Cookies can also always be removed from your computer via the browser.

2. Personal data of potential clients and other interested parties

Data being processed

If you, as a potential client or interested person, contact me, the following personal data may be processed:

  • Your name
  • Your e-mail address (if you e-mail me)
  • Your telephone number (if you call by phone)
  • Personal data that you actively provide yourself

Purpose

I process these data with the purpose of being able to contact you, to provide you with the requested information or to make an appointment.

Legal basis

Your personal data will be processed on the basis of one of the following legal grounds:

  • The permission you have granted
  • The agreed assignment

Storage period

Personal data of potential clients and other interested parties are kept no longer than necessary for the purpose for which the data were collected.

After the first session with you as a potential client, your personal data will be processed like those of clients.

3. Personal data of clients

Data being processed

  • Personal data that you provide during the intake, during the sessions or during a psychological assessment, such as your name, address, telephone number, e-mail, date of birth and details about your health
  • Processing your Dutch national identity number (BSN) only takes place if you want to get reimbursement through your health insurer
  • Personal data that are provided to me at your request by another counselor or referrer

Purpose

These data are processed for the following purposes:

  • To provide treatment, coaching or psychological assessment
  • To maintain contact with you, and
  • To keep a (financial) administration up to date

Legal basis

Your personal data will be processed on the basis of one of the following legal grounds:

  • You have given your permission for processing
  • An agreement or assignment will be carried out at your request
  • I must comply with a legal obligation
  • Data processing is necessary in connection with a vital interest of you as a data subject
  • Data processing is necessary in connection with an interest of me as controller or of a third party, unless this interest conflicts with the interest of the party whose data is processed ánd the interest of the latter takes precedence over the interest of the former

Storage period

I do not keep personal data of clients (in a form that makes it possible to identify the data subject) any longer than is necessary for the realization of the purposes for which they were collected.

I basically use the following retention periods:

  • Medical data, care data and psychological reports with data of a medical nature: twenty years, starting from the time at which they were manufactured, or as much longer as can reasonably be derived from the care of a good care provider or responsible person
  • Psychological reports with only non-medical data: one year
  • Administrative and financial data: seven years

If the retention period of the personal data has expired or if you request destruction before the retention period has expired, the relevant medical data, care data or psychological reports will be destroyed within a period of three months.

Destruction will be omitted if it can reasonably be assumed that:

  • Storing is of great importance for someone other than you as a person concerned
  • Preservation is required on the basis of a statutory regulation, or
  • If there is an agreement between you as the person involved and me

4. Personal data of suppliers and service providers

Data being processed

The following personal data of suppliers and service providers can be processed:

  • Name
  • E-mail address (if you e-mail me)
  • Telephone number (if you call by phone)
  • Personal data that you actively provide yourself

Purpose

I process these data for the following purposes:

  • Administrative purposes
  • Communication about the assignment
  • Issuing an assignment

Legal basis

Your personal data are processed on the basis of the following legal ground:

  • The agreed assignment

Storage period

The personal data of suppliers and service providers are kept for the duration of the agreement and after that only in the financial administration for a maximum of 7 years.

Data provision to third parties

In principle, I will not provide your personal data to a third party other than after your written consent, unless the provision of personal data is legally required and permitted. An example of this is a police request for (personal) data from me in the context of an investigation. In such a case I must cooperate and I am obliged to provide this information.

I can enable service providers (processors) to process personal data that you have provided to me. I use service providers for:

  • Taking care of an online test environment
  • Taking care of the financial administration
  • Managing websites

I never give personal data to service providers with whom I do not have a processor agreement. I make the necessary agreements with the aforementioned processors, so that the privacy of your personal data is guaranteed.

Transfer to third parties outside the EU

I make use of the services of Evenio, based in New Zealand. Evenio manages several websites for me, which are hosted on servers of Dutch hosting providers that comply with the AVG. The privacy of your personal data is guaranteed through a processor agreement.

Security

I have taken appropriate technical and organizational measures to protect your personal data against unlawful processing.

  • This website is hosted on a server of a Dutch hosting provider that complies with the AVG
  • The connection to this website is protected against eavesdropping and manipulation by means of an SSL connection
  • The information you state in your e-mail or on the contact form is sent via a secure SSL connection
  • I use a username and password policy on all my systems
  • I pseudonimize and ensure encryption of personal data if there is reason to do so
  • I make backups of personal data in order to be able to recover these in case of physical or technical incidents
  • I regularly test and evaluate my measures

Your rights concerning your data

You have the right to inspect, rectify or delete your personal data.

You have the right to withdraw permission to process data.

You can object to the processing of your personal data (or a part thereof) by me or by one of my processors.

You have the right to have the your personal data transferred to yourself or by order of you directly to another party.

I can ask you to identify yourself before I can respond to the aforementioned requests.

Changes to this Privacy Statement

I may amend this Privacy Statement from time to time without notifying you. If you want to be informed about the changes, it is advisable to consult this Privacy Statement on a regular basis.

If you have any questions or complaints about the processing of your personal data, do not hesitate to contact me. If you are not satisfied with the outcome of our discussion, you have the right to file a complaint with the Dutch Data Protection Authority (Dutch DPA).
